How Does Sclerotherapy Remove Spider Veins?

Learn how sclerotherapy works to treat spider veins by sealing damaged vessels and rerouting blood flow to healthier veins. Discover what the procedure feels like, recovery expectations, treatment options, and when to seek professional vein care.

How Does Sclerotherapy Remove Spider Veins?

If you've been researching treatments for spider veins, you've almost certainly come across the term "sclerotherapy." It is widely considered the gold standard for treating spider and small varicose veins, but the name itself and the idea of injections can sound intimidating. In this article, we'll explain exactly how this proven procedure works, what the experience is like, and why it remains one of the most effective ways to clear your skin.

The Short Answer

Sclerotherapy works by injecting a specialized solution directly into the spider vein, which irritates the vessel wall and causes it to collapse and seal shut. Once the vein is sealed, blood is naturally rerouted to healthier veins, and your body gradually absorbs the treated vessel, causing it to fade from view over a few weeks.

Why This Happens

The "Sticky" Reaction

The core of sclerotherapy is the "sclerosant," the solution we inject. When this solution touches the inner lining of your vein, it creates a controlled reaction that makes the vein walls sticky. Instead of remaining open like a tube, the walls stick together, effectively closing off the vein.

Starving the Vein

Once the walls stick together, blood can no longer flow through that specific vessel. Without blood flow to keep it open, the vein becomes nonfunctional tissue. Don't worry about your circulation. Your body has countless other healthy veins that immediately take over the workload, actually improving your overall circulation by eliminating these inefficient dead ends.

Natural Absorption

After the vein collapses, your body treats it like a bruise or any other tissue that needs healing. Your immune system slowly breaks down the closed vessel and absorbs it. This isn't an overnight magic trick. It's a biological process that takes time, which is why results appear gradually over 3 to 6 weeks.

Precision Treatment

We use different types of solutions depending on the size of the vein. For tiny spider veins, we use a liquid solution. For slightly larger feeder veins, we often use a foam solution called foam sclerotherapy, which fills the space better and provides more contact with the vein wall for a more effective seal.

Normal vs. Concerning

Normal If:

  • The injection site feels itchy or bug bite like for 15 to 30 minutes
  • You see bruising or redness around the treated area for a week or two
  • The treated veins look darker or worse before they look better
  • You have small bumps or hives at the injection sites immediately after treatment

Call a Doctor If:

  • You have sudden, severe pain or swelling in the leg
  • You develop an open sore or ulcer at the injection site
  • You experience streaking redness or warmth spreading up the leg
  • You have an allergic reaction such as hives or difficulty breathing after leaving the office

How Doctors Evaluate It

At IVY Cardiovascular & Vein Center, Dr. Rishi Panchal and our team don't just start injecting. We first perform a thorough physical exam and often a venous ultrasound. Why? Because spider veins are often fed by deeper, invisible feeder veins.

If we treat the surface spider veins but ignore the feeder veins underneath, the pressure can reopen them or create new ones. Our evaluation ensures we identify and treat the root of the problem, not just the surface symptom, which is critical for long-lasting results.

Treatment Options

Liquid Sclerotherapy

This is the standard for small, flat spider veins, also known as telangiectasia. We use a tiny needle, often smaller than an acupuncture needle, to deliver the liquid solution. Most patients describe it as a series of small pinpricks.

Foam Sclerotherapy

For slightly larger, bluish reticular veins that feed the spider veins, we agitate the solution with air to create a foam. This foam pushes blood out of the way and ensures the medicine touches the entire vein wall, making it highly effective for stubborn feeder veins.

Ultrasound Guided Sclerotherapy

For veins that are too deep to see with the naked eye but are causing surface issues, Dr. Panchal may use ultrasound to guide the needle precisely into the target vessel. This advanced technique allows us to treat complex vein networks safely.

FAQs

Does sclerotherapy hurt?

Most patients report minimal discomfort. You may feel a small pinch when the needle enters and a mild burning or stinging sensation for a few seconds while the solution works, but it fades quickly.

How many sessions will I need?

The number of sessions depends on how many spider veins you have. Most patients need 2 to 4 sessions spaced about a month apart to achieve 80 to 90 percent clearance. One session typically clears about 50 to 80 percent of the treated veins.

Can I walk after the procedure?

Yes. In fact, we want you to walk. Walking immediately after treatment helps pump blood through your deep veins and prevents clots. You can resume normal activities right away, though you should avoid high impact workouts for a few days.

Is it better than laser treatment?

For leg veins, sclerotherapy is generally superior to laser treatment. It is less painful, often more effective for underlying feeder veins, and works faster. Laser treatment is typically reserved for veins too small for a needle or for facial spider veins.

Are the results permanent?

The specific veins we treat are gone forever because your body absorbs them. However, if you are genetically prone to vein issues, you can develop new spider veins over time. Maintenance visits once a year are common.
