Peripheral arterial disease (PAD) is a common circulatory problem, in which build up of plaque (cholesterol, fat, calcium deposits) in the wall of the arteries causes them to narrow. Narrowing of the peripheral arteries prevents adequate blood supply to the limbs, further causing symptoms such as pain, numbness, aching, or heaviness in the leg muscles. In severe cases this will progress to rest pain, wounds, ulcers, and gangrene, a condition called Critical Limb Ischemia (CLI).
The American College of Cardiology includes the following risk factors for PAD:
- Smoking
- Diabetes
- Coronary artery disease
- Kidney Disease
- Uncontrolled blood pressure
- Family History of vascular disease
- Leg pain during exercise that can progress to pain at rest.
- Coldness in one leg/foot compared to the other side
- Change in the color of the leg
- Absence of pulse
- Critical Limb Ischemia (CLI)
- Ankle-brachial index (ABI)

- Aggressive lifestyle modifications

Dr. Rishi Panchal was trained at Yale University Advanced Peripheral Vascular Interventional Program to treat patients with Peripheral Arterial Disease and Critical Limb Ischemia. He brings his skills and knowledge to South Florida to form a Limb Preservation Program to decrease amputation rates and offer an improved quality of life for the patients in his community. He has authored textbooks, received recognition for advanced peripheral vascular work and continues to be a leader in the field.
Critical Limb Ischemia (CLI)
Severe narrowing in the arteries of the lower extremities that causes markedly reduced blood flow, may lead to critical limb ischemia (CLI). CLI is a chronic condition that results in severe pain in the feet or toes. Complications of poor circulation may lead to development of sores and wounds in the legs and feet. In severe cases, if left untreated, CLI may result in amputation of the affected limb.

Advanced minimally invasive procedural options for treatment
- Endovascular procedures are minimally invasive treatment options that deliver successful treatment to PAD and CLI with minimal discomfort and recovery time.
- Angioplasty – a tiny balloon is inserted through a puncture in the groin. A Balloon is then in inflated on or more times as necessary to allow the clogged arteries to open and perfuse with blood flow.
- Peripheral Vascular Stent – placement of a tiny wire mesh tube (stent) in the artery that has previously been opened with balloon angioplasty. Presence of a stent allows for the artery to remain open and adequately perfused with blood.
